HEAD START WORKS!

To apply for Head Start or Early Head Start enrollment you will need to make an appointment with our Intake Specialist Clorinda Lee. Ms. Lee can be contacted at (919) 734-1178, ext. 225.

Office Hours: 

Mon. - Fri. 8:00 a.m. - 5:00 p.m.

You may also download an enrollment application by following one of the links listed below. Be sure to bring the completed application with you to your appointment.

You will need to bring the following items with you to complete the application process:

• Proof of income (W-2 Forms, previous month's check stubs)
• A copy of child's birth certificate
• Social Security cards for parent & child
• Child's immunization records
• Child's current physical with physician's signature (Physicians will need 72hrs advance notice for physical requests)
• Parent and child's current Medicaid or private insurance information
• Verification of special needs (if applicable)
• Food Stamp verification letter and Food Stamp ID number (if applicable)
